Your personal data and special categories of personal data (health data) obtained through the form are processed for the following purposes: establishment and performance of the international health tourism intermediary service contract; conducting preliminary evaluation, analysis, and planning for the requested service (offer, treatment plan, appointment organization, etc.); carrying out activities aimed at increasing patient satisfaction and service quality; following up legal processes and fulfilling requests from official institutions; and sending commercial electronic messages (SMS, e-mail) regarding the Company’s services (provided Explicit Consent is obtained).

3. Categories of Personal Data Processed

The main categories of personal data processed through the form are: Identity and Contact Information (name, surname, phone number, e-mail address, country of residence) and Special Categories of Personal Health Data (complaints, medical history, diagnosis information, details regarding the requested treatment, as declared in the form).

4. Method and Legal Basis for Collecting Personal Data

Your personal data is obtained through entirely or partly automated means (website communication/application form, e-mail, phone call). Our data processing activities are based on the following legal grounds, in accordance with KVKK Articles 5 and 6: Establishment and Performance of the Contract (Article 5/2.c), for the purpose of offering and organizing the health tourism intermediary service and preparing the offer; Explicit Consent of the Data Subject (Article 5/1 & 6/2), for the transfer of health data to health facilities abroad and for marketing communication; and Fulfillment of a Legal Obligation (Article 5/2.ç), for the retention and reporting obligations arising from relevant legislation.

5. Transfer of Processed Personal Data

Your processed personal data may be transferred to the following recipient groups, in line with the purposes stated above and within the framework of KVKK provisions: Domestic / International Transfer (with Explicit Consent): To contracted health facilities, physicians, and specialists from whom you will receive service; Legal Transfer: To legally authorized public institutions and organizations (Ministry, Police, Judicial authorities, etc.) upon request; and Support Transfer: To suppliers providing IT infrastructure support and consulting firms.

6. Rights of the Data Subject (KVKK Article 11)

As personal data owners, you have the right to apply to ADONISMED to: learn whether your personal data is processed; request information if your personal data has been processed; learn the purpose of processing and whether it is used appropriately; request the correction of incomplete or inaccurate personal data; request the erasure or destruction of your personal data; request notification to third parties to whom the data has been transferred; and object to the emergence of a result against you by means of exclusive analysis through automatic systems.
